In order to request a donation from Plaza Lanes, you are required to submit a donation request form at least 30 days prior to your event. All requests will be reviewed by our management team and owners who will then make a determination of agreement. If the the donation is granted we will notify you via email to schedule a date and time to pick your request or mail it to your house. Your email is requred as well as other information listed below.
No Donation Requests will be taken without this application, no exceptions. Donation amounts or packages are subject Plaza Lanes discretion. There is no guarantee your request will be approved. All requests will be evaluated on a first come, first serve basis. By submitting this application form, you agree to these terms.
